How-To Create Your 1094-C

Our last post in this series discussed furnishing 1095-C forms to employees. This post will focus on how to create your 1094-C form. The 1094-C accompanies the 1095-C forms that you send to the IRS, and serves as a summary of the information contained in the 1095-C forms.
The 1094-C also identifies the ALE Member (employer) who is reporting the information return. If the ALE Member is part of an Aggregated ALE Group, the other members of the group must be defined in Part IV of the 1094-C.

You must manually create your 1094-C form within your program. Your HRM software will compile most of the information required for the 1094-C form when prompted. However, there are sections of the form that must be filled out manually. When one or both of your Responsible Officers are certain that everything entered on the form is correct, print and sign it.
You may want to keep a copy of the completed and signed form for your records, along with copies of the 1095-C documents you are reporting. For additional reference, see the 2018 Instructions for Forms 1094-C and 1095-C provided by the IRS.
