In order for us to maintain up-to-date customer contact information, we need your help.

Please fill out this form for any contacts you want to include on your Datatech Customer Support Record. (We have included checkboxes for CPAs and IT Technicians if you would like to have outside vendor contacts noted on your account.)

IMPORTANT: Any contacts we currently have on your account that are not included in this list will be removed from your account.
